The Son Llàtzer Home Hospitalisation Unit has prevented 1,418 hospital stays since it began operating

Hospitalisation at home (HaD) allows patients to receive specialised healthcare without having to stay in hospital. This avoids complications derived from prolonged stays, such as nosocomial infections or functional deterioration. This modality of care is intended for acute patients, chronic patients with exacerbations, post-surgical patients, or those requiring complex treatments such as intravenous infusion of antibiotics or mechanical ventilation, which previously could only be administered in the Hospital.

The launch of this unit reflects the Hospital’s commitment to more personalised, humanised and closer care, and keeps patients in their familiar environment, where they feel better.

During the first six months of activity, 137 patients have been attended, with an average stay of ten days. This has meant a saving of 1,418 conventional hospital stays. The stay per day in HaD represents only 33% of the stay in conventional hospitalisation, which implies a greater efficiency in the use of the Hospital’s resources.

Multidisciplinary professionals

To ensure the proper functioning of the Home Hospitalisation Unit, the Hospital has strengthened its team with the addition of specialised staff. Specifically, three specialists in geriatrics, family medicine and internal medicine, respectively, have been added, all of them new recruits. Eight full-time nurses also work in the unit, as well as a technician in auxiliary nursing care with administrative functions. The multidisciplinary team will work in coordination with the rest of the services at Son Llàtzer University Hospital to provide medical and home care for admitted patients.

Adaptation of a specific area for the service

Son Llàtzer University Hospital has adapted a specific area within its facilities, which includes a care area for the coordination of services and an administrative area for day-to-day management. A logistical system has also been implemented, including two transport vehicles for care staff and a special taxi agreement.

The Home Hospitalisation Unit provides care from Monday to Sunday from 8.00 am to 9.00 pm. To guarantee continuity of care, 061 attends to emergencies from 9 p.m. to 8 a.m., in perfect coordination with the HaD Unit.

A high-quality and efficient system

This new care model is aligned with strategies to optimise the sustainability of the healthcare system, to provide highly complex care with lower costs and better utilisation of hospital resources.
